Now Accepting New Patients – ByWard Family Health Team

Got this email today:

Dear Patient,

We are pleased to share that a new primary care provider will soon be joining the ByWard Family Health Team.

As a result, we will be opening a limited number of new patient spots with a primary care provider.

Care at ByWard Family Health Team is delivered through an interdisciplinary, team-based model, including physicians, nurses, and allied health professionals working together to support your care.

You are welcome to share this opportunity with family members, friends, or others in your network who may be looking for a primary care provider. If you know someone who may be interested, please feel free to forward this email so they can apply.

Eligibility Criteria

To be considered, individuals must:

  • Have valid OHIP coverage
  • Be unattached (not currently registered with another primary care provider)
  • Reside within postal codes beginning with: K1N, K1M, K1K, K1L or K2P
  • Be comfortable receiving care from a male, English-speaking provider

How to Apply

Step 1: Register with Health Care Connect

All individuals must be registered with Health Care Connect. If you are not registered, you may:

Step 2: Complete the BWFHT PCP Request Form

Once registered with Health Care Connect, complete the ByWard Family Health Team Primary Care Provider Request Form. (If the link is no longer active, we have closed registration)

Step 3: Complete New Patient Onboarding Paperwork

Once your request is processed, you will receive an email with next steps, including:

  • Completing the Ministry of Health enrollment form and New Patient Declaration.
  • Reviewing and acknowledging clinic policies

Please note: Individuals will only be considered fully registered and eligible for assignment once all three steps have been completed. Once registered, you will receive confirmation of your assigned primary care provider.

Important Information

  • Processing times may vary based on volume
  • All requests will be reviewed, and applicants will be notified of the outcome.
  • Due to limited capacity, not all requests will result in onboarding
  • We do not provide individual status updates and will not be responding to status inquiries during the review process.

Thank you for helping us connect more patients to primary care.

Warm regards,

ByWard Family Health Team
