I'm not familiar with how the medical field is structured and it sounds like you're saying it may work opposite from how one would expect. I work in Tech sector and typically in order to get a better paying position (career advancement ) we're looking to become more specialized and get more licenses or certifications that employers need, rather than going into a less specialized position that can be done by "anyone".
